Service Agreement

This Service Agreement ("Agreement") is entered into between TEAM DNA LEVEL C BOXING CLUB ("Club") and the undersigned member ("Member") on the terms and conditions outlined below.

Membership Offerings

1. Payment Arrangements:

  • Member may choose month-to-month payments or a 90-day contract for installment.
  • An application fee of $25 is due upon enrollment, which is non-refundable.
  • All month-to-month payments and application fees are non-refundable..

2. Membership Plans:
Plan 1 $216 monthly 12 sessions 5 – 17 years old
Plan 2 $316 monthly 12 sessions 18 years old and up
Plan 3 $289 monthly 14 sessions 5 – 17 years old
Individual $75 hourly Unlimited 1 on 1 training All ages

Benefits of Membership:

  • Focused Training: The newly enrolled trainee will spend 4 to 8 weeks with a coach in a smaller group, enhancing concentration and developing boxing techniques.
  • Age Inclusivity: Our services cater to individuals aged 5 to 99 years, promoting inclusivity and providing a diverse training environment.
  • Affordable Training Packages: Members can choose from various affordable training packages, ranging from $216 to $316 per month, with additional options for individualized training at $75 per hour.
  • Boxing Kits: Members can purchase boxing kits starting from $125, providing essential gear for training.

Refund Policy:

  • Non-Refundable Fees: The application fee and all month-to-month payments are non-refundable.
  • Cancellation of Membership: Members may cancel their membership by providing written notice at least 30 days before their next month’s billing due date.
  • Refund Requests: Refund requests for the 90-day contract for installment will be considered on a case-by-case basis. Such requests must be made in writing, stating the specific reason of cause for the refund request.

Termination of Membership:

The club reserves the right to terminate a membership if a member violates the terms of this agreement or engages in behavior deemed inappropriate or harmful to others.
